SERVICES OFFERED
The individuals we will provide treatment for at the new Hill Street Facility are men and women who are battling addiction, homelessness, poverty and other life problems. We will also be working with Department of Corrections' clients, Drug Court clients, Probation and Parole clients and struggling veterans. Besides our in-patient treatment facility, and our out-patient treatment program, we will be offering Sober-environment housing at all stages of Recovery.
Our housing services include transitional housing and other Recovery-based, structured Sober-living. Our Recovery-living component is designed to help clients benefit from structure and accountability. This environment includes a daily routine with activities centered around 12-step Recovery, education and skill building. Our employment services are committed to helping those acquire the necessary skills that will allow them to find gainful employment and job advancement opportunities. We also network with other groups and businesses to aid in furthering our clients recovery and growth.
The following services will be performed at our Hill Street location:

Crisis Intervention ‐ Performing field interventions in order to stabilize patients and prepare them for care.

Initial Screening ‐ Determine need (CAGE questionnaire) over the phone or in person (staff will perform)

Intake ‐ Attain client history, walk through Orientation, perform Insurance Prior
Authorization, Schedule assessment

Assessment ‐ Biopsychosocial assessment with clinician. Determines level of care, and
treatment schedule

Treatment Plan ‐ Create treatment goals for each individual client during a one-on-one session

Peer Support ‐ Group and individual support sessions

Case Management ‐ Assessing and addressing other issues the client may face, such as housing, legal, employment, etc.

Intensive Outpatient ‐ Conduct group 3 times a week for 3 hours each session. Client education
and process group

Individual Sessions ‐ Conduct individual psychotherapy with clients. Minimum 1 session per week

Discharge Planning ‐ Nearing completion, meeting with client and creating aftercare plan.

Consultation ‐ Meeting regularly with all staff to discuss clients and their issues. Ensuring appropriate
care of clients. Doing weekly case reviews to ensure proficiency of all client records.

Referral ‐ Making appropriate clinical referral for any additional clinical services. Following up
with agencies and organizations collaborating to secure community resources.
